Anxiety disorders are among the most prevalent psychiatric conditions worldwide, yet the pharmacological landscape has changed very little in recent decades. Psilocybin, a naturally occurring indoleamine prodrug rapidly dephosphorylated in vivo to the active metabolite psilocin, has emerged as a mechanistically distinct anxiolytic candidate with notable convergence across preclinical, neuroimaging, and clinical data [ 1 ]. Direct clinical evidence in anxiety disorders remains limited. Two randomized double-blind trials demonstrated that a single high-dose psilocybin session produced large, sustained reductions in anxiety in patients with life-threatening cancerโ€”about 80% maintained clinically significant improvements at 6-month follow-up, with effect sizes exceeding Cohenโ€™s d = 2.0 [ 3, 4 ].

A meta-analysis of six double-blind RCTs ( N = 528) confirmed that acute anxiety adverse events were transient, commonly resolving within 48 hours, while therapeutic anxiolytic effects emerged simultaneously relative to active placebo [ 5 ].
